Well-Being Score Template (Metric 5.2)
This file is the official template for completing Metric 5.2 of the 2026 Excellence Cup Playbook.
The Bottler must use it to detail its well-being initiative, including a description of the initiative, the methodology applied, and the indicator results for 2025 and 2026. After completion, the template must be submitted to the portal as evidence, along with other supporting materials (such as videos, PPT presentations, and other files), ensuring the initiative is registered and validated.
